PRIVACY STATEMENT

Tonguetie Jackie is registered with the Information Commissioner’s Office and in accordance with the new (GDPR) General Data Protection Regulation 2018.
I am a registered nurse and midwife, registered with the NMC (Nursing and Midwifery Council).
I pay my fee every August and revalidate every three years. I am obligated to conduct myself professionally reflecting the Code of Conduct and Standards of Behaviour for Nurses and Midwives.
Information collected: your personal details, including address, phone numbers, NHS numbers, email address; Gp and hospital details; photographs and videos; birth and baby history and both your health and feeding journey so far.
I will ask a reasonably long list of questions before the phone consultation and then more during the phone consultation.
If you notice any errors in the notes, please contact me as soon as possible by phone or email, so that I can correct them.
Can you request a copy of your notes? Yes. Please email me.
Who collects your details? I am a sole trader so the notes are collected and stored by me alone. By phone or message and face to face.
How is it collected? Through our discussions and records made by phone and message and face to face and paper records.
A summary of notes are recorded in babies Red Book. You can share with professionals and family as required.
Why am I collecting the information?
For ongoing records and continuity and reflections of the care of you and baby in your feeding journey.
Your information will not be sold to any other business or shared with colleagues without your knowledge.
The only exception to sharing yours or your baby’s information is if a safeguarding concern is evident and I need to inform social services or the police or other agencies, by law.
Each year I will collect and audit visits, aspects of care and feeding and outcomes.
Stored on a pdf document.
All notes, photographs, videos and documentation will be stored for 25 years according to GDPR.
Who will it be shared with?
You can share with family and professionals as required. Please discuss with me before sharing a copy of your notes or part thereof on any social media.
I will only share with colleagues with your permission to support you further.
I am required by law to report to police and social services if I have concerns re safeguarding.
Consent gives me permission to liaise with health care professionals as needed, to support you. If I ask your permission to use photographs or notes, they will be for teaching or training resources only.
